Eligibility

 Every wellness journey begins with an intake. Prospective patients meet with our intake coordinator where they get a chance to learn details about our programs, rules, and facilities. At this time, we also complete several screenings that aid us with pairing each individual with the program and provider best positioned to meet their needs.

To begin services applicants must possess a valid ID, social security card, and proof of active insurance. Please bring these items to your intake appointment or be prepared to email a copy.

 To remain active in any Away of Change program patients must attend at least one session each month. If a patient goes 30 days without a session, he/she will be considered inactive. If a patient remains inactive for more than 30 days, he/she will be discharged from the program and required to complete a new intake before resuming services.
